Clinical and radiographic assessments improved with MPFL reconstruction
LYON, France — In patients with recurrent patellar dislocation, medial patellofemoral ligament reconstruction improved objective clinical and radiographic assessments, according to data presented here.
“Overall, satisfactory patient-based outcomes were obtained in most patients,” Takehiko Matsushita, MD, said in his presentation at the International Society for Arthroscopy, Knee Surgery and Orthopaedic Sports Medicine (ISAKOS) Biennial Congress. “Pain appears to be the most frequent complication after MPFL [medial patellofemoral ligament] reconstruction, and it should be informed to patients as a possible complication.”
Among 73 knees in 65 patients who received MPFL reconstruction for recurrent patellar dislocation, Matsushita and colleagues found none of the patients reported re-dislocation following surgery. The Crosby and Insall evaluation system showed 40% of knees were rated as excellent, 56% were good and 4% were fair or poor.
“The mean congruence angle was significantly improved from 50.9° to –0.8° postoperatively,” Matsushita said. “The mean Insall-Salvati ratio also significantly changed from 1.1 to 1.0 after surgery.”
KOOS showed significant improvements in the subscales of pain, symptom, activities of daily living, sports and quality of life, according to study results. Matsushita and colleagues found the mean scores for 41 of 42 questions for the KOOS score significantly improved after surgery.
The researchers also found one or more of the five subscales worsened postoperatively in 21% of patients when KOOS scores were assessed individually.
“All 10 patients [with worse subscale scores] were female; however, there was no significant difference in age, radiographic parameter or the ratio of the cartilage injury in the patellofemoral joints,” Matsushita said. – by Casey Tingle
