Arthroscopic superior capsule reconstruction restores superior glenohumeral stability and function

LYON, France — Superior glenohumeral stability and function of the shoulder joint was restored with arthroscopic superior capsule reconstruction, according to a presenter here.

“Arthroscopic superior capsule reconstruction restored superior stability and functional outcome of the shoulder joint with few complications,” Teruhisa Mihata, MD, said at the International Society for Arthroscopy, Knee Surgery and Orthopaedic Sports Medicine (ISAKOS) Biennial Congress.

Prior to arthroscopic superior capsule reconstruction, 3, 6 and 12 months after surgery and yearly thereafter, Mihata and colleagues performed physical examination, radiography and MRI on 72 shoulders in 70 consecutive patients with irreparable rotator cuff tears.

At final follow-up, results showed significant improvement from 31.3 points preoperatively to 92.8 points for the American Shoulder and Elbow Surgeons (ASES) score and from 50.6 points preoperatively to 93.8 points for the Japanese Orthopaedic Association score after arthroscopic superior capsule reconstruction. Mihata also noted a significant improvement in shoulder active range of motion by 55.6° for elevation, by 14.9° for external rotation and by three vertebral bodies for internal rotation.

“Acromiohumeral distance increased from 4.5 mm to 9.5 mm after surgery,” Mihata said.

According to study results, 93% of shoulders had no graft tear or no re-tear of the repaired rotator cuff tendon during the follow-up period. Three months following surgery, Mihata and colleagues found 4.1% of patients with severe fatty degeneration of the infraspinatus tendon experienced re-tear of the repaired infraspinatus tendon, whereas 2.7% of patients experienced a postoperative graft tear at 3 months and 12 months after surgery. – by Casey Tingle
