Large improvements in Physical Component Summary experienced after ACL reconstruction
Patients who underwent unilateral ACL reconstruction experienced large improvements in the Physical Component Summary at 2 years postoperatively that were maintained at 6 years after reconstruction, according to study results.
Researchers evaluated 1,512 unilateral ACL reconstructions performed from 2002 to 2004 in patients currently enrolled in a prospective multicenter cohort. Patients completed the SF-36 preoperatively and at a minimum of 2 years and 6 years postoperatively. Intra-articular pathological conditions and treatment were documented, as well as the ACL reconstruction surgical technique. The researchers then performed longitudinal analysis for the 2-year and 6-year end points.

Overall, the researchers obtained at least one follow-up questionnaire from 93% of patients. Results showed a mean Physical Component Summary (PCS) score of 41.9 points and a mean Mental Component Summary (MCS) score of 51.7 points at baseline. The researchers noted a PCS of 53.6 points and a MCS of 52 at 2 years and a PCS of 54 points and a MCS of 52.4 points at 6 years.
The researchers found a higher baseline PCS score, younger age, lower baseline BMI, 50% or more of the lateral meniscus excised, and no treatment done on a lateral meniscal tear were significant predictors of a higher PCS score, whereas shorter follow-up time since surgery, revision ACL reconstruction, smoking at baseline, fewer years of education and chondromalacia of the lateral tibial plateau were significant predictors of a lower PCS score.
According to the researchers, the mean utility gained at 6 years after ACL reconstruction was 5.3 quality-adjusted life-years. – by Casey Tingle
