Iliotibial band autograft found beneficial in select athletes with FAI

ORLANDO — Arthroscopic labral reconstruction of the hip performed with iliotibial band autograft returned 88% of elite athletes with femoroacetabular impingement to play, according to a study presented at the Arthroscopic Association of North America 2012 Annual Meeting.
“Arthroscopic labral reconstruction of the hip using ipsilateral iliotibial band autograft yields good clinical outcomes, high patient satisfaction and a successful level of return to play in a select group of elite athletes,” Robert E. Boykin, MD, said during his presentation. “Graft integration is seen arthroscopically on magnetic resonance imaging and histologically.”

Boykin and his colleagues performed a prospective study of 26 elite athletes with a mean age of 29 years who underwent athroscopic labral reconstruction using ipsilateral iliotibial band autograft between 2005 and 2010. The cohort included eight soccer players, seven hockey players, four football players, four skiers, one baseball player, one ice skater and one ballroom dancer. The modified Harris hip score (mHHS), Hip Outcome Score (HOS) and SF-12 were among the outcome measurements used in the study.
The mean mHHS score improved from 62 preoperatively to 79 postoperatively. The HOS score improved from 77 to 86. The SF-12 physical component score improved from 42.8 to 47.5 and mental component from 50.5 to 54.7. Of the 26 patients, 23 returned to play.
Boykin added, “This procedure should be considered for a high level athlete presenting with hypoplastic or irreparable labrum, being careful to remember that patients with less than 2 mm of joint space have a high failure rate.”
