Improved clinical scores seen with aperture fixation for MPFL reconstruction

GENEVA — Performing medial patellofemoral ligament reconstructions with an aperture fixation technique for isolated or combined procedures led to significantly improved IKDC and Kujala scores in patients at 2-year follow-up, according to a results presented at the European Society of Sports Traumatology, Knee Surgery and Arthroscopy Congress 2012.

 “[Medial patellofemoral ligament] MPFL reconstruction with aperture fixation significantly improves clinical outcome in isolated, as well as combined procedures,” study investigator Daniel Hensler, MD, of the Trauma Center Murnau, in Murnau, Germany, said during his presentation. The procedure also “shows better results in the setting of an initial surgery when compared to revision surgery,” he said.

Hensler and colleagues studied the preoperative and postoperative outcomes of 50 patients who underwent MPFL reconstruction with aperture fixation.  Eight patients underwent an initial, isolated reconstruction; nine had an initial reconstruction combined with trochleaplasty, osteochondral autograft transfer system (OATS) or distal femur osteotomy; six had a revision, isolated reconstruction and 20 patients had a revision reconstruction combined with trochleaplasty, OATS, distal femur osteotomy or closing of a former lateral release.

Hensler and colleagues noted significant improvements from the overall mean preoperative Kujala and IKDC scores at 2-year follow-up. The scores for patients whose revision MPFL reconstructions were combined with other procedures, however, were significantly less improved than those who had initial, isolated procedures and those whose initial procedures were combined.
