Opening wedge proximal osteotomy with plate fixation similar to chevron osteotomy for hallux valgus

NATIONAL HARBOR, Md. — Using an opening wedge proximal osteotomy with wedge plate fixation demonstrated similar results for treating hallux valgus as seen following chevron osteotomies, according to the results of a prospective, randomized multicentered study.

“Anyone who has performed a proximal chevron osteotomy for this condition will know that it is a challenging operation. We feel that the medial opening wedge offers some ease of surgery convenience and it was worth a multicentered study,” said Mark A. Glazebrook, MD, PhD, MS, at the 2010 Summer Meeting of the American Orthopaedic Foot and Ankle Society, here.

For the study, 75 patients were randomized to receive proximal metatarsal opening wedge osteotomy with plate fixation or a proximal chevron osteotomy. Patient outcomes were evaluated using SF-36 functional scores, the AOFAS Forefoot Metetarsaophalangeal Interphalangeal score, and the Visual Analog Pain Scale, preoperative, and at 3, 6, 12 and 24 months, according to the study abstract.

The researchers found that operating room times were the same for both groups and that clinical outcomes were improved for both groups, Glazebrook said. “The medial opening wedge osteotomy lengthens the metatarsal and the chevron shortens the metatarsal and there were no differences in complications,” he added.

“We think that both osteotomies provide very good clinical outcomes with minimal complications,” he said. “In this study, all three surgeons would suggest that the medial opening wedge provided a more stable construct and a more precise correction.”
