FAI, labral tears can be treated with arthroscopy in adolescents, study finds

Arthroscopic treatment of femoroacetabular impingement and labral tears is both safe and clinically successful in the adolescent population, according to a recently presented study.

Leandro Ejnisman, MD, shared his findings at the 2011 Annual Meeting of the American Academy of Orthopaedic Surgeons, in San Diego.

“The main goal of our study was to report radiographic intraoperative findings, clinical outcomes and revision rates in 64 patients with 70 hips treated for [femoracetabular impingement (FAI)] of hip arthroscopy,” Ejnisman said.

Ejnisman and his group retrospectively analyzed prospective data from 70 consecutive hip arthroscopy patients 16 years of age or younger. Collected information included subjective data, such as the modified Harris hip score and patient satisfaction with outcome graded on a scale of 1 to 10 (10 being most satisfied).

“We excluded patients that had previous hip arthroscopy, patients that had arthroscopy for other pathologies besides FAI, and patients with a center edge angle below 20· — which we considered dysplastic,” Ejnisman said.

Ejnisman reported CAM impingement in 10% of patients, with pincer impingement in 15% of patients and mixed-type impingement in 75%. His group found labral tears in all patients, with a repair rate of 83%. The remaining labral tears were debrided.

The group performed limited femoral osteoplasty in 56 patients, as well as rim trimming in 55 patients. With an average follow-up of 3 years, modified Harris hip scores improved to an average of 93 as compared with a preoperative average score of 60. Median patient satisfaction was 10.

Eight patients — all female — needed revision arthroscopy. Median satisfaction among this group following revision was 9.

“Arthroscopic treatment of FAI in the adolescent population is a safe procedure,” Ejnisman concluded. “Clinical outcomes and patient satisfaction were excellent 2 years postoperatively.”
