Acetazolamide improves vision in patients with idiopathic intracranial hypertension

Acetazolamide in combination with a weight-reduction diet slightly improved vision in patients with idiopathic intracranial hypertension, according to a study. The Idiopathic Intracranial Hypertension Treatment Trial, a multicenter, randomized, double-masked study, included 161 women and four men with idiopathic intracranial hypertension and mild vision loss. Eighty-six patients received acetazolamide, and 79 patients received a placebo. Dosage was up to 4 g daily. All patients were on a low-sodium weight-reduction diet.

A 54-year-old man was transported to the emergency room from a rehabilitation facility with sudden onset double vision, left-sided weakness and right-sided facial droop. The patient had been recovering after a recent hospitalization for subacute endocarditis and a cerebellar cerebrovascular accident when he noticed sudden onset horizontal double vision that was present in all fields of gaze, inability to close his right eye and left-sided weakness.

A 51-year-old man with a history of hypertension and ulcerative colitis complained of acute blurriness in the lower half of his right visual field upon awakening during hospitalization for an ulcerative colitis flare-up.
