Keep hyaluronidase on hand to address injection filler complications

KOLOA, Hawaii — Hypersensitivity reactions to injection fillers are rare and unpredictable, but when they occur, clinicians need to know how to manage them, a presenter here said.

“Remember that when you’re injecting, know what you need to do when you run into complications,” Jennifer Murdock, MD, said at Hawaiian Eye 2023.

Lip fillers are becoming increasingly popular, she said.

“Over 2 million people had them in 2018. It is a huge market. There are now 17 [hyaluronic acid] fillers in the U.S. and more and more keep coming out,” Murdock said.

When a patient is given lip filler, swelling is expected. Even so, to be prepared for the unexpected reaction, clinicians should have hyaluronidase on hand to dissolve the filler, Murdock said.

Additionally, oral prednisone, ice and Benadryl may help reduce the reaction.

One thing to consider before giving any filler is whether a COVID vaccination has been administered recently to the patient, Femida Kherani, MD, FRCSC, said in related panel discussion.

“One thing I do ask is not to have any COVID vaccinations 2 weeks to 4 weeks before any filler” to decrease risk of a sensitivity reaction, Kherani said.
