After cornea heals, consider LASIK if enhancement needed following cataract surgery

WAIKOLOA, Hawaii — Cataract surgery is a journey, with certain steps surgeons should follow, according to a presentation at Hawaiian Eye 2022.
“The road you travel on starts with the evaluation and the surgery, and then the last three steps are the YAG, the neural adaptation and the possible fine-tuning,” Michael Greenwood, MD, said.
Most patients are happy after cataract surgery, but surgeons need to be ready to help patients who are not satisfied.
Regarding timing, it is important for everything to heal before performing any adjustments. Corneal healing from cataract surgery takes about 3 months, Greenwood said.
Greenwood discussed results from a retrospective chart review examining patients who underwent LASIK or PRK enhancement after cataract extraction.
“In our practice, the two most common ways to [fine-tune] are LASIK and PRK,” he said.
Three hundred thirty (67.2%) of the patients who underwent LASIK achieved 20/20 uncorrected distance visual acuity or better compared with 142 (42.9%) of the patients who underwent PRK.
“If you’re choosing between LASIK or PRK and the patient is eligible for both, LASIK seems to be a little bit more predictable and have a little bit better outcome when comparing it to PRK in our practice,” Greenwood said.
