Forgiving optic, stable haptics of IOL matter in challenging cases

In the experience of Andreas F. Borkenstein, MD, the Carl Zeiss Meditec CT Lucia IOL is safe to implant in challenging cases, such as pseudoexfoliation syndrome, that are more at risk of complications during and after surgery.
“Lucia has a lot of features that matter in challenging cases,” Borkenstein said at the European Society of Cataract and Refractive Surgeons virtual meeting.
During surgery, phacodonesis and decentration commonly occur in eyes with pseudoexfoliation, and this is addressed with Lucia by the ZO optic concept, designed to compensate for a range of aberrations arising from different corneal shapes and lens misalignment.
“The optic combines the benefits of aspheric and neutral IOLs, making the lens less sensitive to decentration. This is not only important in PXF cases, but also in other challenging cases such as high myopia, uveitis, previous trauma or vitreoretinal surgery, retinitis pigmentosa, diabetes and all connective tissue disorders that lead to zonular insufficiency,” Borkenstein said.
Other common surgical complications in these eyes are phimosis and subluxation, for which the design of the haptics is important.
“The optic haptic junction of Lucia is thicker, wider and stiffer, and this in our experience provides increased stability,” Borkenstein said.
Because of the thicker optic haptic junction and haptics, Lucia is slightly more difficult to fold and press through the incision site, but the 621P injector is specifically designed to host and smoothly release this lens.
“An easy-to-use, reliable injector is important in dealing with other challenges commonly occurring in these eyes, such as poor pupil dilation, floppy iris, bad visibility and a thinner, fragile posterior capsule,” Borkenstein said.
The IOL is also good in standard cases, according to Borkenstein.
Of 53 patients implanted with this IOL, 92% were within 0.5 D of target refraction, and 100% were within 0.75 D, and the posterior capsular opacification rate is encouragingly low, he said.
