Some of the happiest doctors I work for labor in solo or nearly solo practices, settings where business affairs are comparatively simple and one's control can be absolute. The death knell for solo practices was rung loudly starting a little over 30 years ago, when I first started consulting. Today a new generation of worrywarts assume that the end of the boutique, mom-and-pop practice is upon us. Not so fast. Small and nimble outfits often can, with effort and intelligence, deliver a unit of patient care for far less cost than their mega-jumbo competitors, in settings where there can be a frustrating diseconomy of scale and ponderous decision making.
