The video showcases the challenging scenario of capsular bag-IOL complex subluxation with a capsular tension ring.

The entire bag-IOL complex was grasped with end-opening forceps, and the capsule that surrounds the IOL and the CTR was removed with a vitrectomy probe. As a three-piece IOL was present, the tip of the same haptic was held and externalized as in a glued IOL surgery. The CTR was removed with a closed chamber approach, and single-pass four-throw pupilloplasty was performed to reshape the pupil.
