Phakic lens in high myopes yields early refractive stability

A 3-month prospective study carried out at a Chinese university showed safety, efficacy and stability of results with the implantation of a posterior chamber phakic ICL in highly myopic eyes.
The Staar Surgical ICL V4c has a central hole that enhances aqueous humor circulation to avoid cataract formation and IOP increase with no need to perform peripheral iridotomy. The authors noted that there is a lack of data on the implantation of this lens in China, where high myopia is highly prevalent.
In total, 67 eyes of 38 patients were implanted with the ICL V4c, 30 of them with the toric model. The mean preoperative spherical equivalent was –12.44 D, ranging between –6.63 D and –20.5 D. The mean cylinder was –1.69 D, ranging between –0.25 D and –5 D.
At 1 month and 3 months, all eyes had corrected and uncorrected distance visual acuity of 20/40 or better, and 92.54% were within +1 D of the intended spherical equivalent refraction. The mean spherical equivalent improved to –0.3 D.
“The refractive parameters, including visual acuity and manifest refraction, showed identical results at 1 and 3 months postoperatively, suggesting that patients’ refractive status could be stabilized soon after ICL V4c implantation,” the authors wrote.
Optical quality, measured using a double-pass optical quality analysis system (OQAS II, Visiometrics), was high and stable from shortly after implantation. The mean modulation transfer function cutoff was 38.2 cycles per degree, and the mean objective scatter index was 1.
The mean IOP was 15.3 mm Hg preoperatively and 14.19 mm Hg 3 months after implantation.
“The stable IOP indicates that the central hole in ICL V4c could help maintain the IOP without peripheral iridotomy, which further certified the safety of ICL V4c implantation,” the authors wrote. They also noted that, as of yet, “no study has reported secondary cataract after ICL V4c implantation.” – by Michela Cimberle
