LOS ANGELES — Adding a toric platform to a multifocal IOL significantly improves vision at intermediate and near distances, according to one speaker.

“Our goal was to show that, if you put the toric platform on a multifocal, are you going to get just as good vision or is it going to worsen the vision putting those two things together? In this intermediate [distance], it actually improved the vision one line by correcting the astigmatism,” Stephen V. Scoper, MD, said at the American Society of Cataract and Refractive Surgery meeting.

In a prospective study, 386 patients were implanted with the AcrySof ReStor (Alcon) multifocal toric IOL with 3.0 D added power and 188 with the nontoric multifocal AcrySof with 4.0 add power. Patients in the first group had cylinder between 0.75 D and 2.82 D, while in the other group cylinder was less than 0.75 D.

At 12 months, mean binocular uncorrected intermediate vision for the toric multifocal group was at least one line better than for the nontoric group and distance-corrected vision was at least 1.5 lines. Near acuity, both monocular and binocular, uncorrected and distance-corrected, tended to be better, although in a nonstatistically significant proportion. The binocular defocus curve showed the full range of vision for both IOLs and was not negatively affected by the toric component.

“Both lenses perform well at all distances, and it was important to prove that near vision is not harmed by adding a toric platform to a multifocal optic,” Scoper said.

The combination of toric and multifocal powers is of interest to many people, because since approximately 65% of patients undergoing cataract surgery have astigmatism between 0.25 D and 1.2 5D, which may limit their visual quality after surgery, he said. – by Michela Cimberle
