Rate of retinal detachment after bag-in-the-lens IOL implantation similar to rate after traditional implantation

The incidence of rhegmatogenous retinal detachment after bag-in-the-lens IOL implantation corresponds to the rate observed after lens-in-the-bag IOL implantation, according to a study.

The prospective cohort study evaluated 1,323 consecutive cases of bag-in-the-lens IOL implantations for a mean follow-up of 44.75 months.

There were 19 cases of postoperative rhegmatogenous retinal detachment (RRD) throughout the follow-up, with five cases occurring within the first year. At the 2-year follow-up, the cumulative incidence of RRD was 0.84%.

Male gender (P = .002), patients younger than 60 years of age at the time of surgery (P = .004), axial length of 25 mm or greater (P < .001) and history of contralateral retinal detachment (P < .001) were preoperative risk factors significantly correlated with RRD occurrence after bag-in-the-lens implantation.

Without those preoperative risk factors, excluding male gender, the 2-year cumulative incidence of RRD decreased to 0.15%.

“Further multicenter prospective research will remain important in determining the exact incidence of this late complication,” the study authors said. “We suggest a prospective study set-up in a cohort in which analysis is presented first with all risk factors included and then a subanalysis with risk factors excluded, with the exception of gender, to allow a more thorough comparison of studies.” – by Kristie L.
