Large-area application of MMC during trabeculectomy more successful

A large surface area application of mitomycin C during trabeculectomy attained a higher IOP reduction and a reduced rate of bleb scarring compared with eyes treated with a small area of application, according to a study.
One hundred ninety-one eyes were included in the retrospective study; 96 eyes underwent trabeculectomy with small-area MMC application and 95 eyes underwent trabeculectomy with large-area MMC application.
IOP was significantly reduced in both groups during follow-up visits, but the decrease was significantly greater in the large-area MMC group compared with the small-area MMC group at 14 days, 4 weeks, 3 months, 6 months and 12 months postoperatively.
However, aggressive MMC use led to choroidal detachment, shallow anterior chamber and bleb leak.
Bleb failure complications such as bleb scarring were more frequent in the small-area MMC group.
The large-area MMC group had an overall higher success rate at 12 months postoperatively.
