Formulas unsuitable for predicting PVR after retinal detachment surgery

Formulas that predict development of proliferative vitreoretinopathy after retinal detachment surgery were not accurate enough for regular clinical use, according to a study.

The prospective study included clinical variables from 1,047 patients with primary rhegmatogenous retinal detachment who participated in the Retina 1 Project, which was conducted in Spain and Portugal. The data were used to validate four previously published formulas, F1 to F4.

Investigators used receiver-operating characteristic curves to validate the quality of the formulas and measures of discrimination, precision and calibration. They also determined agreement between formulas.

Accuracy was 74.21% for the F1 formula, 58.9% for F2, 54.59% for F3 and 44.75% for F4.

Specificity was 77.99% for the F1 formula, 60.26% for F2, 53.15% for F3 and 44% for F4.

F1 incorrectly classified almost 19% of proliferative vitreoretinopathy cases; 13% of cases were incorrectly classified by F2, 15% by F3 and 10% by F4.

There was moderate agreement between F2 and F3 but little agreement between the other formulas.

To increase usefulness of the formulas, genetic variables may need to be included, the authors said.
