WAIKOLOA, Hawaii — Success is achieved with proper preparation, a presenter told colleagues interested in learning what to expect when implanting a trabecular micro-bypass shunt for the first time.

“Planning for the operating room is extremely important,” Malik Y. Kahook, MD, said at Hawaiian Eye 2013.

Such as with other fine surgical maneuvers, implanting the iStent (Glaukos) requires skill and finesse, Kahook said. For those new to the procedure, Kahook suggested planning, practice, patience and appropriate patient selection.

Preparation is essential, including relearning and practicing gonioscopy, if necessary. First patients should be cooperative and have open angles, and first cases should be scheduled in blocks, preferably at the end of the day, when there is less concern about falling behind schedule, according to the presentation.

“Take your time and schedule five to seven cases before making any assumptions or decisions about the implantation technique,” Kahook said.
