A single-piece IOL positioned posterior to the iris resulted in markedly greater anterior chamber depth than a three-piece implant, according to a study.

The prospective, randomized study included 40 patients with a mean age of 74 years who underwent phacoemulsification and in-the-bag implantation of a single-piece Tecnis ZCB00 IOL (Abbott Medical Optics) in one eye and a three-piece Tecnis ZA9003 IOL (AMO) in the other eye.

Study results showed that mean anterior chamber depth was 4.21 mm in the single-piece IOL group and 3.94 mm in the three-piece IOL group. Mean iris-to-IOL distance was 0.7 mm in the single-piece IOL group and 0.44 mm in the three-piece IOL group.

Between-eye differences were statistically significant for anterior chamber depth (P = .002) and iris-to-IOL distance (P < .001).
