Spectral domain OCT may detect macular thinning in primary open-angle glaucoma

PHILADELPHIA — Spectral domain optical coherence tomography successfully measured macular thickness in patients with open-angle glaucoma and unilateral visual field loss, a speaker said here.

"Unilateral glaucoma and spectral domain OCT remains a promising model system for further longitudinal studies and may be able to define macular changes in the first affected eye," Camilla S. Zangalli, MD, said at the Wills Eye Institute Annual Conference.

The prospective study included 14 patients with primary open-angle glaucoma and unilateral visual field defects. A control group comprised 13 healthy subjects. Inclusion criteria were best corrected visual acuity of 20/40 or better, no opacity and no refractive errors. Patients with cataracts, corneal or retinal disease, or any condition associated with visual field loss were excluded.

Investigators used the Spectralis spectral domain OCT system (Heidelberg Engineering) to measure the thickness of the retinal nerve fiber layer (RNFL), ganglion cell complex (GCC) and ganglion cell-inner plexiform layer (GCIPL).

Results showed that mean macular GCC thickness decreased inferiorly from 90 µm to 66 µm and temporally from 74 µm to 60 µm. Mean GCIPL thickness decreased inferiorly from 55 µm to 43 µm and temporally from 57 µm to 38 µm. All decreases were statistically significant (P < .001).

GCC and GCIPL thicknesses were not affected superiorly or nasally.

RNFL thickness decreased from 190 µm to 63 µm in the inferior region and from 107 µm to 85 µm in the superior peripapillary region. The decreases were statistically significant (P < .001), Zangalli said.
