Zigzag cut ideal for femtosecond DALK

ROME — A zigzag cut performed with a femtosecond laser enhances stability and fast healing of deep anterior lamellar grafts, according to one surgeon here.

"The zigzag incision acts like a 'floating boat dock' so the anterior edges of the donor and recipient line up nicely compared to a straight vertical incision where the thickness of the donor and recipient never perfectly match and sometimes leave significant size mismatches," Francis W. Price Jr., MD, said at the annual joint meeting of Ocular Surgery News and the Italian Society of Ophthalmology.

The zigzag cut also minimizes air escape into the peripheral cornea during big bubble formation and is easier to suture.

"The use of femtosecond laser with this technique might ease the transition from [penetrating keratoplasty] to [deep anterior lamellar keratoplasty] for many surgeons who are still holding back due to the difficulty of the manual approach," Dr. Price said.
