Trypan blue from compounding pharmacy recalled
RICHFIELD, Minn. — Potential bacterial contamination has led to the recall of trypan blue 0.06% ophthalmic solution prepared by a compounding pharmacy in the Midwest. The recall affects seven states and the District of Columbia.
Custom Rx Compounding Pharmacy said in a press release that it is issuing a nationwide recall of the solution because of potential contamination with Pseudomonas aeruginosa. Use of the contaminated vials should be stopped immediately, the release noted.
The trypan blue formulated by Custom Rx was distributed to hospitals and clinics in Illinois, Maryland, Michigan, Minnesota, Nebraska, North Dakota, Pennsylvania and the District of Columbia, according to the company. The solution is dark blue in appearance and is packaged in 1 cc sterile tuberculin syringes, according to Custom Rx.
The recall includes but may not be limited to lot numbers 05042005:86@17, 05252005:36@13. 06282005:91@27, 08012005:63@24 and 08182005:43@17.
The pharmacy is voluntarily recalling the product based on two reports of vision loss possibly associated with the use of the product as reported by the Centers for Disease Control and a positive bacterial culture obtained from an outside hospital.
