ROME — Ab interno trabeculectomy with the Trabectome compares favorably with standard trabeculectomy, according to a study carried out in Greece.

The Trabectome device (NeoMedix) is inserted through a corneal incision — the same that is used for cataract surgery in combined procedures — and trabeculectomy is performed ab interno, under a gonioscopic lens that visualizes the trabecular meshwork.

"We compared the results of 67 eyes that underwent this procedure with 44 eyes that underwent standard trabeculectomy," Theodoros Filippopoulos, MD, said at the winter meeting of the European Society of Cataract and Refractive Surgeons.

The Trabectome procedure seemed to have a better safety profile; it produced fewer complications and fewer early IOP spikes. A more rapid visual rehabilitation was obtained.

"This should be the procedure of choice in patients on three medications requiring cataract surgery or in younger patients with good vision," Dr. Filippopoulos said.
