Study: Performing separate trabeculectomy, ECCE provides better IOP control

MUNICH, Germany — Performing a trabeculectomy 1 month before extracapsular cataract extraction provided better results than performing both at once, according to a study presented here.

Anas Nameh, MD, of Syria, compared the postop results of 40 eyes of 40 patients with both cataract and open-angle glaucoma who underwent extracapsular cataract extraction (ECCE) or ECCE combined with trabeculectomy.

Dr. Nameh said patients were divided into three groups. Group 1 comprised 14 patients who underwent ECCE and trabeculectomy at one time. A second group of 12 patients underwent trabeculectomy first and ECCE 1 month later. The third group of 14 patients underwent ECCE alone, Dr. Nameh said here at the European Society of Cataract and Refractive Surgeons meeting.

Patients in group 2 had the best overall control of IOP. Additionally, these patients showed less inflammation postop and the lowest incidence of complications overall.

After a 3-month follow-up, patients in the first group had a mean IOP reduction of 10 mm Hg, those in the second group had a mean reduction of 13 mm Hg and those in the third group had a mean reduction of 4.6 mm Hg.
