Study finds limited benefit from needling revision with MMC of failed Molteno implants

Needling bleb revision with mitomycin-C can be used to manage failed encapsulated Molteno tube implants, according to a study by researchers in Iran. However, the effectiveness of the technique may be limited, the study authors said.

Reza Zarei, MD, and Saeid Shahhosseini at the Farabi Eye Research Center, Tehran University of Medical Sciences, evaluated outcomes for 16 eyes of 16 patients that underwent needling bleb revision with mitomycin-C (MMC) at a mean age of 28.8 years. All eyes had an encapsulated bleb around a Molteno drainage device and medically uncontrolled high IOP, according to the study.

The researchers found that needling revision reduced IOP from an average of 28.25 mm Hg preoperatively to 16.69 mm Hg at 3 months and to 21.13 mm Hg at 6 months follow-up. Success was achieved in 87.5% of patients at 3 months, in 37.5% at 6 months and in 12.5% at 24 months of follow-up, the authors reported.

The study is published in the February issue of Asian Journal of Ophthalmology.
