Study: Combination phaco-trabeculectomy in primary angle-closure glaucoma requires no further surgery

BOSTON — Half of the primary angle-closure glaucoma cases in a study that received trabeculectomy alone needed further surgery within 3 years, while cases that received both trabeculectomy and cataract surgery required no further surgery.

Catherine Jui-ling Liu, MD, spoke at the joint symposium of the Japan and Taiwan Glaucoma Societies, held at the World Glaucoma Congress here, about the retrospective study. She also spoke regarding the use of phacoemulsification alone in treating primary angle-closure cases. She said that phaco alone can achieve IOP control in some patients.

However, phaco combined with trabeculectomy is still necessary for certain cases.

"Phaco-trab is indicated for eyes with advanced [primary angle-closure glaucoma] which have preop IOP greater than 35 mm Hg while on maximum medications," Dr. Liu said.

She and colleagues examined 99 eyes with primary angle-closure glaucoma. Of those, 24 eyes received trabeculectomy, and 75 underwent combined surgery. At 3 years follow-up, 13 eyes (54%) in the trabeculectomy group required phaco or additional surgery (P < .001). No eyes in the combined group required further surgery.
