RLE, phakic IOLs show comparable retinal detachment rates

ALICANTE, Spain — Refractive lens exchange and phakic IOL implantation have identical outcomes in terms of retinal detachments, according to evidence from the literature.

Emanuel Rosen, MD, said here at the Alicante Refractive International meeting that the annual retinal detachment rate in the general population is 0.012%. That rate is 10 times higher after cataract extraction with phacoemulsification. In myopic eyes, the rate of retinal detachment is naturally much higher, and increases to 2-8% after phaco, he said.

Phakic IOLs have similar indications to refractive lens exchange in high myopic eyes. Dr. Rosen noted that in his experience of implanting more than 500 eyes with phakic IOLs, only one developed a retinal detachment.

“This seems to coincide with the results of other studies published in literature, where the rate of retinal detachment after implantation of phakic IOLs is around 2%,” he said.

“In both procedures surgical invasion of the anterior segment does produce potential posterior segment consequences in large eyes, but advantages still outweigh disadvantages. With the necessary information both methods can be offered to selected patients,” Dr. Rosen said.
