Preoperative astigmatism correlated with clinically significant corneal haze after PRK without mitomycin C.

The findings suggested that patients with moderate or high preoperative astigmatism of 1.25 D or more should receive prophylactic MMC during conventional PRK procedures.

The retrospective study included 362 eyes of 193 patients who underwent PRK.

Data showed that nine eyes of six patients had grade 2 corneal haze or greater. Of those eyes, five eyes had more than 2 D of astigmatism and four eyes had no clinically observable risk factors. The correlation between postoperative corneal haze and level of preoperative astigmatism was statistically significant (P < .001).

"To our knowledge, this is the first study to demonstrate a significant association between corneal haze and high astigmatism treatment with conventional PRK," the study authors said. "We believe that the shape of the ablation and the acute rate of change in curvature in astigmatism treatment contribute to corneal haze formation."
