ROME — Posterior corneal analysis is essential to rule out false positive forme fruste keratoconus, according to a specialist.

"Keratoconus in its natural history progresses from posterior to anterior. In many cases, topography shows that posterior changes occur before anterior steepening," Ming Wang, MD, said at the annual joint meeting of Ocular Surgery News and the Italian Society of Ophthalmology.

New technologies are extremely sensitive, and detecting even small changes in anterior corneal regularity, thickness and curvature may lead to incorrect diagnosis. Whenever an anterior irregularity is detected, the posterior cornea, where the disease starts, should be investigated.

"If there are no corresponding posterior changes, an ectatic disease is unlikely to be involved," Dr. Wang said.
