Phaco challenging but possible in chronic angle-closure patients

NEW DELHI — Phacoemulsification is possible in eyes with chronic angle-closure glaucoma and a functioning filtering bleb, according to a study conducted here.

Researchers with the Institute of Medical Sciences conducted a prospective, noncomparative study of cataract surgery in 60 eyes of 44 patients with angle-closure glaucoma and a functioning filtering bleb. Patients had stable intraocular pressures of 21 mm Hg or lower for at least 6 months prior to surgery. The surgeons performed phaco with in-the-bag IOL implantation.

Iris trauma was the most common complication, seen in 11 eyes (18%). Corneal edema was seen in seven eyes (12%) and breach of rhexis was seen in two eyes (3%). Vitreous loss, fibrinoid reaction and increased IOP were seen in one eye each (2%).

The report appears in the August issue of Clinical & Experimental Ophthalmology.
