Oral ciprofloxacin levels effective in silicone-filled eyes

NEW DEHLI, India — Effective levels of drug can be achieved in silicone-filled vitrectomized eyes with oral administration of ciprofloxacin, according to a study here.

After vitrectomy, antibiotic levels of ciprofloxacin in the retro-silicone oil space fluid in silicone oil-filled eyes exceeded the MIC90 of isolates for most bacterial species, the study found.

Dinesh Talwar and colleagues assessed the penetration of oral ciprofloxacin into the vitreous in four groups of eyes. They gave one dose of 750 mg ciprofloxacin to two groups of five patients with vitrectomized eyes with silicone oil endotamponade. Group 1 received the drug 4 hours before silicone oil removal and the second group 8 hours before removal. A third group of 10 patients with vitrectomized eyes with silicone oil and a fourth group of 10 scheduled for vitrectomy received two 750 mg doses, 24 hours and 12 hours before surgery.

The mean drug concentration in the retro-silicone oil space fluid ranged from 0.34 to 0.84 µg/mL, with the highest concentration in group 3. Mean serum concentrations were not statistically significant between groups 3 and 4. The levels of serum in the vitreous were higher in non-vitrectomized eyes.

The study is published in the February issue of Investigative Ophthalmology & Visual Science.
