OCT study reveals more definitive picture of juvenile X-linked retinoschisis

The schisis cavity associated with juvenile X-linked retinoschisis may appear most frequently in the inner nuclear layer and at the outer nuclear layer/inner plexiform layer, yet morphologic appearance is more varied than commonly thought, according to a study.

In a study of 34 eyes of 18 male patients diagnosed with juvenile X-linked retinoschisis examined with spectral-domain optical coherence tomography, the schisis cavity was visible at the inner nuclear layer (INL) in 29 eyes and at the outer nuclear layer/inner plexiform (ONL/OPL) layer in 22 eyes.

However, retinal schisis was also visualized at the retinal nerve fiber layer (RNFL) in four eyes. Widespread foveomacular involvement was common, although schisis was confined to the fovea only in nine eyes of six patients.

"Contrary to previous histopathologic reports, involvement of the superficial retina (RNFL and [ganglion cell layer]) was seen only rarely," the study authors wrote. "Based on this analysis, the foveomacular schisis in juvenile X-linked retinoschisis seems to occur predominantly at the INL, occasionally at the ONL/OPL, and only rarely at the RNFL."
