Oblique side cut penetrating femto-keratoplasty shows good results

BUDAPEST, Hungary — Penetrating femto-keratoplasty with oblique side cuts, using the VisuMax femtosecond laser from Carl Zeiss Meditec, provides fast recovery with low risk of tissue rejection due to reduced endothelial surface, according to one surgeon.

"By using a 110° oblique side cut, the amount of transplanted endothelial surface can be reduced while increasing the diameter of the stroma and Bowman membrane," Georges Baikoff, MD, said at the winter meeting of the European Society of Cataract and Refractive Surgeons.

Side cuts, he noted, are easy to perform, and the donor and recipient are self-matching. Only eight superficial interrupted cardinal sutures and one deep running suture are used to hold the graft in place, with minimal induction of astigmatism.

"The visual results are excellent, and in 70% of cases, postoperative astigmatism before suture removal is less than 4 D. Mean endothelial cell count is 1,980 per mm²," he said. "Because of these results, I have switched from lamellar keratoplaty to [penetrating femto-keratoplasty] with 110° side cuts in all keratoconus patients."
