Needling revision useful in glaucoma management

Needling revision of a failed bleb is useful for IOP management in glaucoma, according to a Scandinavian retrospective study.
Marit Fagerli and colleagues reviewed the records for 26 eyes that underwent needling revision after a failed trabeculectomy. The revisions were performed either without antimetabolites or with adjunctive use of mitomycin C or 5-fluorouracil. Mean follow-up was 14.5 months.
Postoperatively, IOP decreased from a mean of 28.8 mm Hg to 15.3 mm Hg. The success rate after initial needling was 64% at 6 months, a rate that was maintained at the 1 year and 2 year follow-up visits. For patients that had one or more needling, success was 96% at 6 months and 77% at years 1 and 2.
