Multifocal lens exchange ‘best’ for many presbyopic patients

MAUI, Hawaii — One of the “best treatments” for presbyopia is to use the AMO Array multifocal lens in a lens exchange, according to Tom M. Coffman, MD.

“Successful results dictate excellent reproducible biometry and practice in cataract patients with AMO’s SI40 lens,” Dr. Coffman said here during Hawaii 2003: the Royal Hawaiian Eye Meeting.

In performing the lens exchange, the surgeon should structure the incision and other operative factors to end up with 0.5 D or less of cylinder. The IOL haptics should be either both in the bag or both in the sulcus. If placed in the bag, the haptic tips should be oriented to the 12-o’clock and 6-o’clock positions.

Dr. Coffman said to avoid patient complaints of nighttime glare, the surgeon should operate on the second eye soon after the first. In the interim, he advises surgeons to inform their patients that someone else should be responsible for driving at night.
