Low myope wants full correction for outdoor activities
The 57-year-old patient underwent LASIK with the Bausch & Lomb Zyoptix.

History: A 57-year-old man presented for LASIK consultation. He was a low myope with excellent functional vision indoors, but he desired a full correction because he is an active hunter with bow and arrow. When counseled regarding treatment options, he chose a custom wavefront correction. I report the results of the right eye at 3 months after surgery, as the results of both eyes are very similar.
Preoperative examination:
Uncorrected visual acuity: 20/80; high contrast logMAR: 0.85
Manifest refraction: —1.00 –1.00 × 65
Visual acuity: 20/20; high contrast logMAR: 0.04; low contrast logMAR: 0.32

Postoperative refraction:
Uncorrected visual acuity: 20/20; high contrast logMAR: 0.12
Refraction: Plano –0.50 × 10
Visual acuity: 20/20+; high contrast logMAR: 0.04; low contrast logMAR: 0.24
Higher-order aberrations (6-mm pupil): Reduced from 0.61 µm to 0.36 µm
Vertical coma: Essentially unchanged (from 0.05 µm to 0.08 µm)
Horizontal coma: Reduced from 0.18 µm to 0.03 µm
Spherical aberration: Reduced from –0.45 µm to –0.14 µm
Discussion: This case demonstrates the usefulness of custom treatment even in cases of low myopia with relatively low preoperative higher-order aberrations. The Snellen acuity data indicate an appropriate result expected even from a standard laser treatment. However, the improvements of low-contrast acuity and higher-order aberrations are usually not seen after standard treatment, especially in the case of spherical aberration. The patient said he was very happy with his postoperative visual quality when hunting.
