Prediction and Control of Corneal Asphericity After Refractive Surgery

To the Editor:
In the article by Cantu et al, which appeared in the September/October 2005 supplement to the Journal of Refractive Surgery, the authors proposed an ablation algorithm to increase spherical aberration and corneal asphericity (Q) to create an improved prolate ablation profile and the corresponding modulation transfer function (MTF) in myopic and hyperopic patients. A related article by Lee et al2 also proposed a two-step algorithm for the improvement of overall visual performance, in which they use the standard Munnerlyn formula for plano correction followed by a second step to eliminate high order aberration without removing too much extra corneal tissue, a contrasting strategy to the zone enlargement method, which suffers the risk of a thin remaining corneal bed.
