LASIK using custom wavefront ablation yielded fewer higher-order aberrations, better predictability and higher contrast sensitivity than conventional ablation, a study showed.

The prospective, nonrandomized case series included 239 eyes that underwent LASIK with conventional or custom ablation for myopia or myopic astigmatism. The LADARVision excimer laser (Alcon) was used in all cases.

The group included 89 eyes that underwent conventional LASIK and 150 eyes that underwent custom ablation LASIK.

Primary measures were refractive outcomes, ocular higher-order aberrations and contrast sensitivity.

At 6 months postop, the custom ablation group had a mean spherical equivalent of –0.03 D and the conventional ablation group had a mean spherical equivalent of –0.14 D. The difference was statistically significant (P = .003).

Study data showed custom ablation yielded more predictable results, with 99% of eyes in the custom ablation group and 92% of eyes in the conventional ablation group within 0.5 D of intended correction. The difference was not statistically significant. However, higher-order aberrations were statistically significantly lower in the custom ablation group (P < .001).

The custom ablation group also had statistically significantly better contrast sensitivity than the conventional ablation group, the authors said.
