IOP fluctuations alone not a risk factor for glaucomatous visual field loss

In patients with ocular hypertension, IOP fluctuations were not an independent risk factor for the incidence of glaucomatous visual field loss, according to a Swedish study.
Boel Bengtsson and Anders Heijl at Lund University examined 90 patients already enrolled in the Malmo Ocular Hypertension Study every 3 months with office-hours diurnal tension curves and computerized perimetry. Patients were followed for at least 10 years or until glaucomatous visual field loss could be demonstrated.
After 17 years, 37 patients had developed glaucomatous visual field defects. Mean IOP of all measurements during the prospective part of the study was a significant risk factor for developing glaucoma, while IOP fluctuations were almost significant. IOP fluctuation depended linearly on IOP level, meaning IOP fluctuation was greater in eyes with higher IOP.
The study is published in the June issue of Graefe’s Archive for Clinical and Experimental Ophthalmology.
