Hyperopic PRK with MMC prevents haze, endothelial cell loss

Intraoperative topical mitomycin C prevented haze formation in eyes undergoing hyperopic PRK.

“Although the prophylactic effects of MMC on haze in myopic surface ablation have been widely published, no previous data on MMC in hyperopic PRK are available except a report of an atypical case after keratoplasty,” the study authors said.

The prospective, randomized study included 88 eyes of 88 patients that underwent PRK with intraoperative MMC, and a control group (91 eyes of 91 patients) that underwent PRK with balanced salt solution. Both groups had a mean age of about 39 years.

Mean preoperative best corrected visual acuity was 0.06 in both groups. Mean preoperative endothelial cell density was 2,342 ± 540 cells/mm² in the MMC group and 2,298 ± 581 cells/mm² in the control group.

All PRKs were performed with the Technolas 217z excimer laser (Bausch & Lomb). Postop examinations were conducted at 2, 3, 4, 6 and 30 days, and 2, 4, 6, 12 and 18 months.

At 18 months, mean BCVA was 0.06 in the MMC group and 0.08 in the control group. Also at 18 months, haze was 0.05 in the MMC group and 0.23 in the control group. No endothelial damage was seen in either group. Epithelialization was complete within 6 days without adverse outcomes in all cases, the authors said.
