Good results seen with combined MICS, implantation of glaucoma device

ROME — Patients undergoing combined cataract and glaucoma surgery can benefit from simultaneous implantation of the Ex-PRESS miniature implant under the scleral flap, according to one surgeon speaking at the European Society of Cataract and Refractive Surgeons winter meeting.

In a study carried out at Bassano Hospital Eye Clinic by Simonetta Morselli, MD, and colleagues, the safety and the efficacy of the procedure were evaluated in two groups of 10 patients. In the first group, patients underwent combined trabeculectomy and microincision cataract surgery. In the second group, MICS was combined with implantation of the Ex-PRESS implant (Optonol) under the sclera flap. The mean age of patients was similar in the two groups, and the mean follow-up was 9 months.

In both groups, a significant and comparable reduction of IOP was achieved without additional medical therapy.

The group in which the Ex-PRESS implant was used had fewer transient complications related to IOP fluctuations immediately after surgery.

"In our series, the Ex-PRESS device seemed to reduce complications and surgical time, but of course there are higher costs involved in this procedure," Romeo Altafini, MD, said.

No cases of infection, corneal erosion or Ex-PRESS extrusion were observed. A longer follow-up is needed to confirm these results.
