DSEK triple procedure yields better visual outcomes but more complications than sequential DSEK

PHILADELPHIA — Study results showed better visual outcomes for a combined Descemet's stripping endothelial keratoplasty procedure, but early data suggested that sequential DSEK resulted in fewer complications, a speaker said here.

Of 54 eyes that underwent DSEK after cataract surgery alone (sequential DSEK), six (11%) experienced graft dislocation and one (1.9%) experienced graft rejection, Priscilla G. Fowler, MD, said at the Wills Eye Institute Alumni Conference.

Of 50 eyes that underwent a combined DSEK, cataract extraction and IOL insertion (DSEK triple procedure), eight (16%) experienced graft dislocation, four (8%) experienced graft rejection and one (2%) failed completely.

Data were reviewed from records collected since 2007 at a single institution.

"We recognize that our study is limited by the fact that we have a smaller number of uncomplicated combined procedures available for review, yet we feel that the data demonstrate potential trends that warrant further study," Dr. Fowler said.
