Detachment rate reduced after presoaking DSEK donor grafts in balanced salt solution

Detachment rate was reduced to 0% in 20 grafts presoaked in a balanced salt solution before Descemet's stripping endothelial keratoplasty.
In a retrospective, interventional case series of 103 eyes undergoing Descemet's stripping endothelial keratoplasty (DSEK), the final 20 procedures differed in that donor tissue was presoaked in BSS Plus (Alcon). Of the eyes in the first 83 procedures, 25 (30%) experienced detachment afterward; of the final 20 eyes with presoaked grafts, none did. All procedures were performed at one institution by two surgeons.
"The results suggest that presoaking donor tissue in BSS Plus lowers the detachment rate of DSEK grafts," the study authors said.
The BSS solution acts as a diluent in the presoaked tissue, removing detrimental agents through diffusion, which may promote graft adherence, the study said. The solution contains an antioxidant, which helps maintain barrier function of corneal endothelial cells, and glucose, a source of energy for those cells.
