The different chromosomal mutations responsible for corneal dystrophies should be taken into consideration when clinicians choose treatment strategies, according to Eun Suk Lee, MD and Eung Kweon Kim, MD, PhD.

LASIK can aggravate corneal deposits in Avellino corneal dystrophy and should be avoided in patients with that condition, the authors report. Phototherapeutic keratectomy (PTK) has been shown to be useful in clearing opacities with visual improvement and preventing painful erosion, resulting in delay or postponement of corneal grafting in some corneal dystrophies. Mitomycin-C, when used in conjunction with PTK, can help reduce the recurrence of opacities, the authors said.

The review of surgical strategies is published in Current Opinion in Ophthalmology.
