Spherotoric bag-in-the-lens IOL implantation results were in line with implantation of other spherotoric IOLs, even in eyes with ocular comorbidity or irregular astigmatism up to 15°, a study found.

The study included 52 consecutive eyes of 35 patients with cataract and corneal astigmatism ranging from 0.9 D to 6.19 D who had undergone spherotoric IOL implantation with a bag-in-the-lens technique. One-third of the eyes had an additional ocular comorbidity; 12 had high myopia and five had high hyperopia. Of the eyes, 5.2% had irregular astigmatism up to 15°.

Eyes were assessed at 1 day, 1 month and 1 year postoperatively. The researchers found that spherotoric IOL implantation using the bag-in-the-lens technique yielded comparable results to other accepted techniques.

The study authors recommended correction of corneal astigmatism at the level of the IOL plane, including in eyes with comorbidity or astigmatism up to 15°.

"In these cases, patients should be informed that their corneal astigmatism will not be fully corrected but that their dependence on spectacle independency will likely decrease," they said.
