Articaine shows more effect than lidocaine plus bupivacaine for peribulbar anesthesia
Articaine 2% without hyaluronidase was more advantageous than a mixture of lidocaine 2% and bupivacaine 0.5% without hyaluronidase for peribulbar anesthesia in cataract surgery, according to a Turkish study.
M. Ozdemir, MD, and colleagues at Kahramanmaras Sutcu Imam University compared the two anesthetic approaches in 58 patients undergoing cataract surgery. Ocular and eyelid movement scores, number of supplementary injections, total volume of solution used and pain/complications were used as endpoints.
Articaine produced greater akinesia after 5 minutes. Eighteen patients in the articaine group (60%) and 26 in the lidocaine/bupivacaine group (93%) required a second injection. A third injection was required by two patients in the articaine group and 12 in the combination group. Median pain score was lower in the articaine group than in lidocaine/bupivacaine group during injection and surgery. The incidence of complications was not different between the groups.
