SAN DIEGO — Children with aphakic glaucoma may be more prone to early IOP elevation after secondary IOL implantation, a presenter said here.

"Patients with aphakic glaucoma are required to have more meticulous ophthalmic viscosurgical device removal," Rupal H. Trivedi, MD, said at the American Society of Cataract and Refractive Surgery meeting.

The retrospective chart review assessed the incidence of postoperative day-1 IOP elevation in 133 eyes, defining postop spike as an IOP of greater than 26 mm Hg.

A spike occurred in 7% of eyes. Twenty-seven percent of eyes with aphakic glaucoma experienced elevation, compared with only 3% of patients without aphakic glaucoma.

"We are now considering more aggressive use of topical and systemic glaucoma medications during [the] early postoperative period," Dr. Trivedi said.

The reported incidence of IOP elevation may be an underestimation because postop IOP is not routinely measured in all patients, she said.
