Somatus shows hospital readmissions down 49% among patients with CKD

Patients with kidney disease under the care of Somatus transitioning back home after a hospital stay were 49% less likely to be readmitted to the hospital compared with patients who did not receive the care, the company reports.
Validation Institute, a company that reviews and confirms performance claims made by health care providers, reviewed data from 14,628 health encounters provided to patients with chronic kidney disease who were managed by Somatus under its Transitions of Care Assessment (TCA) program.
“As we continue to deliver our whole-person care model at scale, our care teams are doing the necessary work to keep our patients from experiencing unplanned readmissions and remain healthier at home,” Joe Kimura, MD, chief medical officer at Somatus, said in a press release.
Somatus, founded in 2016, has established value-based partnerships with nephrology and primary care groups, health plans and health systems to provide integrated care.
Previous analyses of Somatus patient data included patients in stages 4 and 5 CKD; the new analysis by Validation Institute includes patients in stage 3 CKD, the company said.
TCA services post-hospitalization provided by Somatus include care coordination, medication management, nutrition and social services. Services ideally begin within 72 hours of hospital discharge with the goal of improving health, safely transitioning to home and avoiding another hospital stay, according to the Validation Institute report.
“As the TCA program outcomes were calculated based on claims data, there may have been additional factors that could explain additional variation or improve propensity matching to non-participants,” according to the report. “However, improvements due to TCA and non-participant readmission rates were consistent with industry published estimates. Ongoing sensitivity analyses are being conducted to incorporate individual comorbidities in composite predictors of readmission risk specific to the kidney disease patient population.”
