Somatus launches new program with Inova Health System to improve kidney care

Somatus announced in a press release the launch of the Nurse Navigator Program with its partner Inova Health System.
The Nurse Navigator Program is designed to give patients more face-to-face interactions with registered nurses and registered nurse care managers. According to the release, this design is aimed to educate and empower patients while reducing readmissions and length of stay in acute care settings.
Additionally, it is intended to improve the transition of care after hospitalizations so that patients attend necessary appointments and adhere to prescribed care.
“The program is an extension of care for the nephrologist and primary care physician,” according to the release.
In January, Inova Health System implemented the program across all five hospitals.
“The Nurse Navigator Program is an excellent example of our shared commitment to innovation coming to life,” Ikenna Okezie, MD, co-founder and CEO of Somatus, said in the release. “Together, we will continue to provide exemplary quality care and improved outcomes for all our patients.”
